Referrals Refer Referrals can come from any source such as a family, a GP or hospital, as long as the child’s parent or carer with parental responsibility has given their consent. We welcome referrals of children and young adults with life-threatening and life-limiting conditions in London who meet our acceptance criteria. Before you make a referral please speak with your GP or consultant who can support you in gathering the clinical information you require to support your referral. To make a referral download and then complete this referral form along with up to date clinical information (you must have one of the following: clinic letter or discharge letter) and either post it to us at the address at the bottom of this page or email it to our care team. Referrals are assessed at weekly multi-disciplinary meetings and families and / or referrers will be contacted within seven days with a decision. If the child or young person meets the criteria and is accepted onto our caseload, the care team will contact the family to arrange a tour of the hospice. After this a member of our residential care team will work with you to create a care plan for your child, documenting the needs of your child and family and preparing medical documents. Next, a day care session or overnight stay is arranged. If we agree an overnight stay then we will ask you to spend the night in your child’s room or in one of our family flats. We expect you to only stay with your child for the first stay so that your child can get to know us and the care team have the opportunity to get to know the care needs of your child and family After that we will be in contact with you to arrange further care and support that is bespoke to your family’s needs.
